The College provides credit cards through JP Morgan/Chase Bank for College related travel and entertainment and non-routine minor purchases.
Request Credit Card
To request a credit card, please complete the necessary information on this form.
Once you submit, the Financial Accountant will receive an email and forward your request to your department head and to the Vice President of Finance and Administration for approval. Once all approvals have been received it will take 5-7 business days for your card to be received by the Business Office and we will contact you to pick up your new credit card.
Access Credit Card Statement
To access your credit card statement on line, please follow this link.
Please code all credit card transactions with the proper account number and provide a brief explanation. By the 15th of each month please submit a copy with your supervisor approval of your prior month statement and original supporting receipts to the Business Office.
Lost Credit Cards
If you have a dispute on your credit card statements please contact JP Morgan Chase directly at 888-297-0768 or 800-380-5540. For lost or stolen credit cards, please call 1-800-316-6056. In addition please inform the Business Office.
